What is in YOUR IFAK

Up until I took Max Velocity's TC3 class Battlefield Triage my IFAK was basically a few band aids, aspirin, etc.

Here is what is in my IFAK now.

> 2 Tourniquet, Combat Application 1 in the top of my IFAK and 1 on the left suspender
> Israeli Compression Bandage
> 2 Kerlix, roll
> 2 Adhesive Tape Surg 2”
> Airway, Nasopharyngeal 1 (+small tube of lube)
> 2 Pair Gloves
> Combat Gauze
> 2 adhesive chest seals.
> ARS for Needle Decompression ‘Pen’ (14 gauge x 3.25 in.)
> Tough cut scissors on a lanyard tied to the inside of the IFAK

I bought ALL of the above things from Amazon.

During the TC3 training Max taught to the acronym MARCH...

M Massive Hemorrhage 
A Air way
R Respiratory
C Chest wound
H Hypothermia

No application of fluids via intravenous on the battle field. Leave that up to the professionals. 

He also taught that if possible, the person "hit" should put on his suspender located tourniquet high and tight and then get back into the fight to support his team.

I carry a V.O.K. kit that tactical response put together aswell as trauma sheers, C.A.T. tourniquet, Combat gauze, alcohol wipes, and an assortment of pills. Pain, electrolyte etc.

I joined uwgear & MV's forums with USMC0331 and am using it in  the future to get away from the negative "prepper" connotations and from being "boxed in" by the title.

Since a few here post on both, I thought it would lesson confusion.

Long story short. I went all over creation here in RI/SE MA. to find this stuff. At my final stop I talked to my towns Sr. EMP guy.

He told me that he buys all of the EMP supplies on Amazon as the big supply houses expect him to buy in qty. The place I ordered my ARS for Needle Decompression ‘Pen’ had 90% of the items I needed. The company was North American Rescue.

Don't have everything yet but I'm planning to have

For hemorrhage
-h&h compressed gauze
-Celox gauze
-2 SOFT wide tourniquets

For tension pneumothorax
-14 gauge decompression needle
-halo chest seals

For airway
-nasopharyngeal airway
-(possibly) some Vaseline

Other
-gloves (even though I'm totally clean, other people might be squeamish)
-emt shears

I only have theoretical knowledge on how to use this so I'm trying to keep it simple and only have 1 or 2 things for each likeley scenario. Band aids and Advil would be kept seperate from the life saving stuff.

A few things I feel i need to mention.

To those that are prepared to perform a chest needle decompression, keep in mind that there is one of the ICA's Intercostal Artery directly below the 2nd rib (and all others)which will require further surgery if damaged. So try and stay as close to the top of the 3rd as you can.

I forgot to list that I also keep an ammonia inhalant crush tube in my kit. I've seen it wake up "unresponsive" people that did not budge with pain or noise stimuli.

And for the love of god those with the nasal tubes have some lube for that. I was the "volunteer" for the demonstration during my medical training and the medic was fresh out of lube. which had to be one of the most uncomfortable experiences in my life. (And by uncomfortable I mean painful as fuck.)

Yeah I hear ya' Burt. Mine came with a ketchup type foil wrapped lube - but that was mine. Remember you are not supposed to use yours so be a buddy and carry some with yours.

When I took my class we were also told we could use spit or fresh blood if your injured buddy didn't have any lube.

The gloves aren't aren't for the vic Grudgie, they are there to protect you. Lord only knows what kinda hepCherpies these people have meow a days.

1 roll medical tape
1 window punch

Med Bag contains all those items in bulk plus - blind insertion airways, BVM, suction, various boo-boo supplies, IV equipment, splinting supplies, triage tags, diagnostic equipment and meds.

The IFAK has a few more added items than normal yes. I've added the CPR pocket mask with the thought process of having to provide artificial ventilations in the warm & cold zones. The most likely scenario being someone going into cardiac arrest at any point during day to day life (not talking about CPR under fire, because that's an obvious no-no.) The two adult sized OPA's work in conjunction with the cardiac arrest mindset as they hold a better expedient airway than NPA's in unresponsive individuals with no gag-reflex. (My opinion).

The two 14g angiocaths because one of two reasons. First being, often times initial decompression will require a second decompression just lateral of the initial decompression. The soft catheter can occlude itself after time in the harder tissue of the thoracic cavity. Secondly, if the rescuer is unsure of which side the tension pneumo is on, having two angiocaths allow bilateral placement. So one is bound to be on the correct side. Without ascultaing for diminished lung sounds, it can be a guessing game as to which side the tension pneumo is located.

Two tourniquets because often times one doesn't stop the hemorrhage and a second TQ is required to be placed proximal to the first.

A little extra gauze because...well you can never have to much gauze!

Window punch, because it takes up minimal space and gives you easy access to injured individuals in vehicles.
Space blanket because people often forget this simple shock intervention; keeping the patient warm. Blood loss + heat loss = death. Prevent additional heat loss.

I like to use the MARCH acronym.

Massive Hemorhage
Airway
Respirations
Circulation
Hypothermia
